The primary indication for a total laryngectomy is advanced laryngeal cancer. This cancer can originate in different parts of the larynx, including the glottis, supraglottis, and subglottis. When the cancer is too extensive to be treated with less invasive methods like radiation therapy or partial laryngectomy, total removal becomes necessary. Other indications include recurrent cancer after initial treatment and severe trauma or damage to the larynx.
